The Senior Registered Nurse (RN) with a BSN independently observes, assesses, plans, intervenes and evaluates outcomes and documents all aspects of patient care. Coordinates scheduling within assigned areas and provides primary management for service area during the assigned shift with supervisory support. Supervises, counsels, and evaluates other nursing staff. Collaborates with the Nurse Coordinator, other charge nurses, Psychologist, Physicians and Mental Health Supervisor to ensure 24-hour continuity of care. May assume duties of the Nurse Coordinator/Nurse Manager during absence. Provides direct nursing care to individual patients as needed. Duties, location of work and shift/pattern are subject to change as determined by administration to meet hospital needs. Duties may include exposure to blood or other potentially infectious materials. Performs other duties as assigned, including supporting the agency's disaster response and/or Continuity of Operations (COOP) activation. This may require alternate shift patterns or locations. This position may be eligible to earn additional pay for work performed on evenings, nights and/or weekends. Essential Job Functions (EJFs): Attends work on a regular basis and may be asked to work a specific shift schedule or rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned. Assess the health/mental status of the patient on admission and ongoing basis and document assessment, planning, intervention, and evaluation in the patient record to determine priorities and plan therapeutic nursing care. Evaluates patient data and medical history to assist providers with establishing treatment plans. Interpret and evaluate patient response to plans of care. Performs CPR/Heimlich techniques according to American Heart Association specifications. Provides direction and coordination of staffing and patient care activities on assigned units, addressing performance problems promptly and maintaining accurate documentation of interventions. Coordinates and administers effective utilization of nursing service personnel on assigned units. Facilitates smooth admission/transfer processes and communicates with admissions staff to ensure appropriate status and triage patients when needed. Works with providers and other health care professionals to establish and implement courses of treatment. Follows provider orders to deliver and document individualized plans of care. Carries out medical staff practitioners\' orders and implements nursing interventions per policies and procedures. Communicates diagnosis and treatment plans to patients, family members and doctors. May assist in abuse/neglect investigations. Implements special precautions as ordered and documents rights restrictions; maintains patient safety. Attends mandatory training to ensure staff competence and development and participates in ongoing in-service education. Monitors treatment, intervention and protection strategies (TIPS) to ensure well-being and compliance with program requirements during restraints where applicable. Communicates with leadership to disseminate current information to staff. Performs other duties as assigned, including disaster response/COOP participation with possible alternate shifts or locations. Knowledge, Skills and Abilities (KSAs): Knowledge of state and federal laws for individuals with mental health issues, rules, regulations, policy and procedures related to State Hospital. Knowledge of the Texas Nurse Practice Act and Board of Nursing rules. Knowledge of emergency procedures and equipment (CPR) and ability to respond in emergencies. Knowledge of nursing care standards, the Settlement Agreement and Healthcare Guidelines. Skills in supervising employees and in providing nursing care with appropriate tools and equipment. Ability to lead change, analyze problems, and develop policies and procedures. Proficient in computer use and electronic health records. Strong time management, prioritization and communication skills. Ability to build collaborative relationships and train staff. Ability to interpret regulations and develop improvement plans. Ability to organize, coordinate and evaluate nursing activities, and to instruct and guide others. Ability to maintain continuing education hours. Registrations, Licensure Requirements or Certifications: Licensed to practice as a registered nurse (RN) in Texas (or state with reciprocity via the Nurse Licensure Compact) prior to hire. If transporting patients, must have a valid Texas driver license or obtain within 90 days. Out-of-state license requires driving record and compliance with HHSC Fleet Management policy. Initial Screening Criteria: Graduation from an accredited nursing program or four-year college/university with nursing major; at least three (3) years of RN experience in a recognized health care facility. Additional Information: Applicants must pass fingerprint background check, pre-employment drug screen, and registry checks (CANRS, Nurse Aide, Medication Aide, Employee Misconduct, LEIE). Males 18-25 must register with Selective Service. All applicants must be 18+; some locations may require 21+. Flexibility in hours; overtime may be required.
