PROGRAM AND LOCATION

VOA, a Certified Great Place to Work, is recruiting for a full‑time Service Coordinator position for Sierra Manor, located in Reno, Nevada. Sierra Manor provides safe, HUD‑subsidized housing for Reno seniors 62 or over. Full‑time employees (working 30 or more hours a week) are eligible for a benefits package that includes paid holidays, vacation, sick pay, health, dental, vision, life, and short‑term disability insurance coverage. POSITION SUMMARY

The Service Coordinator is responsible for developing, implementing, and maintaining a case management plan including assessments, coordination of community services, follow‑up and monitoring for individuals residing in Volunteers of America affordable housing programs. Work is performed in a cost‑effective and service‑oriented manner as it relates to established organizational standards. OBJECTIVES/ACTIVITIES

Perform daily work requirements to achieve established program objectives. Provide general case management (including intake) and referral services to all residents needing such assistance. Establish partnerships with public and private agencies such as AAA’s/ADRC’s, hospitals, health systems, and primary health providers. Routinely assess service needs in response to changing circumstances, consulting with residents returning from the hospital/rehabilitation to determine need for additional support. Conduct annual assessments for all residents participating in the program. Provide formal case management for residents when such service is not available through the general community. Establish links with community agencies and service providers to determine the best individualized, flexible, and creative services for the involved residents. Assemble a directory of community services and providers and make it available to residents, families, and management. Refer and link residents to service providers in the general community, including case management, personal assistance, homemaker, home‑delivered meals, transportation, counseling, occasional visiting nurse, preventive health screening/wellness, and legal advocacy. Monitor the delivery of services to residents to ensure they are appropriate, timely, and satisfactory. Meet with service providers as needed and appropriate. Report all suspected abuse situations to the appropriate agency. Provide training to project residents in tenancy obligations or coordinate such training. Educate residents on service availability, application procedures, client rights, and advocacy. Create volunteer support programs with community service organizations. Help residents build informal support networks with other residents, family, and friends. Educate the management team on aging in place and service coordination. Ensure that service provider costs do not exceed residents’ ability to pay. Develop service plans with residents to address needs and interests identified through assessments (a plan is required for all frail residents). Ensure all residents have access to a Primary Care Provider. Perform administrative and clerical duties. Document contact with residents, providers, and families and follow up on all outreach and case management activities. Maintain individual files on residents, including psychosocial information, short and long‑term plans to maintain and improve self‑sufficiency. Complete documentation and reports to comply with contractual agreement and VOA requirements, including client demographic statistics, regulatory and funding source requirements, support services provided, and referrals accepted. Pursue avenues for additional services through private, local, state, and federal sources. Perform other reasonable related duties as assigned by Community Administrator. EDUCATION AND EXPERIENCE

A bachelor’s degree in social work, gerontology, psychology, counseling, public health, or therapeutic recreation is preferred. At least two years of experience conducting supportive service needs assessments and using such assessments for the elderly to identify and locate specific services that address individual residents’ needs is required. Demonstrated working knowledge of supportive services and resources for senior citizens and/or non‑elderly people with disabilities available in the local area. Knowledge of federal and state entitlement programs, legal liability issues related to providing service coordination, and the ability to advocate, organize, problem‑solve, and achieve results for the elderly and people with disabilities is essential.
