Job Description
Partner with the Nursing Resource Coordinator, Care Transitions team, Center for Patient Flow, and all units of UAB Hospital to facilitate early discharges.
Coordinate the patient care experience for clinically cleared patients awaiting discharge completion or additional services such as transportation.
Assume responsibility for technical aspects of patient care as directed by the RN/LPN.
Assist with activities of daily living (ADL) as needed.
Obtain and record vitals, weights, inputs, and outputs.
Escalate any patient care need beyond the scope of the role.
Complete the in‑house PCT training program.
Demonstrate awareness of patient and personal safety.
Perform administrative tasks in support of patients while assigned to the Discharge Lounge, including coordinating with ambulance services, Care Transition team, and Food & Nutrition Services to arrange transportation, medications, and nutritional support.
Qualifications
High school diploma or equivalent.
Five (5) years of patient care experience.
Successful completion of the in‑house PCT training program.
Current Cardiopulmonary Resuscitation (CPR) certification or to be obtained within thirty (30) days of employment.
