Full Job Summary Plans, organizes, and implements Physical Therapy services for PACE participants.

Essential Duties and Responsibilities

Provides high quality care as a member of the PACE interdisciplinary team (IDT).

Performs in‑person assessments for participants at initial enrollment and as needed.

Assesses the need for adaptive durable medical equipment, specialized shoes, prosthesis, and home safety and mobility; recommends home modifications and equipment.

Plans and administers physical therapy treatment programs according to recognized standards and policies.

Documents evaluation, assessment, plan, and treatment rendered accurately and in a timely manner.

Measures effectiveness of treatment, documents and revises plan accordingly.

Educates participants/family in procedures, home programs, and documents training and understanding.

Trains PACE staff in body mechanics, use of equipment, transfers, gait patterns, and methods to maximize safety and functional abilities.

Provides care and services in the center, clinic, contracted facilities, and participants’ homes according to each participant’s plan of care.

Assists participants to function at the most independent level possible.

Maintains positive relationships and communication with co‑workers, participants, families, and the community.

Collaborates with the interdisciplinary team to develop a comprehensive care plan for each participant.

Attends and participates in daily interdisciplinary team meetings and other related meetings.

Performs duties of other IDT members as needed according to licensure and competence.

Participates in quality improvement projects and meetings.

Completes all documentation of clinical service in participants’ medical records.

Follows all policies, procedures, OSHA safety guidelines, privacy and confidentiality requirements, and standard precautions.

Maintains a safe working environment and participates in continuing education and required training.

Maintains professional affiliations, certifications, and continuous education requirements.

Other duties as assigned.

Experience, Education, and Certifications

Advanced degree in Physical Therapy from an accredited school.

Current license to practice Physical Therapy in the state.

Current driver’s license with proof of auto insurance.

Two (2) years of experience on an interdisciplinary team in a hospital, nursing home or community setting preferred.

One (1) year of experience working with frail or elderly populations preferred; training will be provided if not present.
