We are seeking a compassionate
Registered Nurse
to join a growing
Managed Long-Term Care (MLTC) team . In this role, you will coordinate care, conduct in-home assessments, and support older adults in living safely and independently.
Key Responsibilities
Conduct comprehensive in-home assessments and reassessments of members’ health and functional status.
Develop and maintain individualized care plans tailored to each member’s needs.
Coordinate and authorize covered services across providers and payers.
Communicate and collaborate with members, caregivers, and interdisciplinary teams to ensure optimal outcomes.
Serve as a clinical advocate, promoting member well‑being and continuity of care.
Maintain accurate documentation and compliance with MLTC guidelines and policies.
Requirements
Active NYS RN license & BSN.
Minimum 3 years RN experience; MLTC or geriatrics experience preferred.
What We Offer
Salary: $105,000–$115,000 + $20,000 sign‑on bonus.
Hybrid schedule: 4 days remote / 1 day field visits (Brooklyn office + Manhattan field work).
