RN Case Manager — North Metro Home Care
Location: St Louis Park, MN (Minneapolis and surrounding suburbs)
Pay: $38.00–$42.00 per visit

Essential Job Functions
Conduct comprehensive assessments and develop individualized care plans.
Provide nursing care, medication administration, and health education.
Monitor and reassess patient needs, adjusting care plans accordingly.
Collaborate with physicians, families, and support staff.
Supervise PCA/home health aides and mentor new RN Case Managers.
Maintain accurate documentation and fulfill certification/recertification requirements.
Participate in on‑call duties and quality improvement initiatives.
Requirements
Active Registered Nurse (RN) License
Two‑Step TB Test (we provide testing if needed)
DHS Criminal Background Clearance (we provide screening)
Valid Driver’s License & Auto Insurance
