Home Health Occupational Therapy Case Manager
Prairie River Home Care, Inc. is seeking a Home Health Occupational Therapy Case Manager to provide skilled OT services to clients in their homes and support their recovery and daily living activities.
SCHEDULE
Full time, Monday‑Friday with an on‑call rotation.
Allow flexibility to manage schedule during office hours.
Travel to clients’ homes for 1‑3 hour visits per client.
COVERAGE AREA
Reports to the Buffalo, MN office and serves clients in Maple Grove, Brooklyn Park, Brooklyn Center, and surrounding cities.

Major Responsibilities
Administer occupational therapy evaluations and participate in the development of the Plan of Care.
Establish and maintain treatment goals, and reassess clients to make recommendations.
Provide occupational therapy treatments, procedures and diagnostic tests as directed by physicians.
Translate exercises into functional activities for daily living.
Provide instructions for special equipment and modify client environments.
Prepare clinical and progress notes and report client response to physicians and Area Clinical Manager.
Maintain records of visit frequency and client response.
Instruct and supervise COTAs, HHAs, LPNs and RNs in the therapy program when appropriate.
Deliver in‑service education to company employees as requested.
Participate in case conferences.
Offer families information, support, and education to motivate client progress and facilitate home therapy.
Assist families in adjusting positively to limitations.
Develop community resources for clients.
Maintain positive and effective communication with all employees.
Safeguard client confidentiality.
Maintain a safe client environment and report suspected abuse or neglect.
Maintain productivity as per guidelines.
Perform other related duties as assigned.
Physical/Environmental Demands
Lifting, bending, stooping, stretching, and equipment setup; use of force up to 100 lbs occasionally and 20 lbs constantly.
Adaptable to various environments; must have reliable transport.
Qualifications
Licensed Occupational Therapist in Minnesota, no revocations, suspensions, or restrictions.
Graduate of an AOTA‑approved OT school and currently certified by the AOTA.
Minimum of two years experience in a healthcare setting (home care preferred).
National Board certification strongly recommended.
Strong interpersonal, teaching, organizational, written, and oral communication skills.
Dependable vehicle or public transportation for multi‑stop travel; valid driver’s license and proof of insurance.
Proof of negative Mantoux or negative chest X‑ray.
Current CPR certification recommended.
U.S. citizenship or valid Alien Work Permit.
Disclosure of any convictions related to health services; convicts will not be hired.
Successful completion of background checks and screenings, including Minnesota Department of Health and the Federal Office of the Inspector General’s List of Excluded Individuals and Entities.
Salary
Base salary range: $70,000 - $95,000 per year plus productivity bonuses and benefits.
