Advanced Practice Provider – Nurse Practitioner or Physician Assistant
Join our team at ConcertoCare. We are seeking an Advanced Practice Provider – Nurse Practitioner or Physician Assistant to deliver home and community based care to patients with complex needs.
Our mission is to provide seniors and older adults with a holistic, equitable, and compassionate approach to health and wellness. Our tech-enabled in-home care teams leverage a value-based interdisciplinary model to improve patients’ quality of life.

Responsibilities
Provision of compassionate, individualized, and holistic patient care including comprehensive care, urgent care, and chronic condition management focused on gaps in care, patient risk stratification, and integration with multidisciplinary team services.
Collaboration with the multidisciplinary team to cooperatively deliver transitions of care support, social determinants of health interventions, behavioral health evaluation and management, and complex medication management as clinically appropriate.
Delivery of clinical care both in-person and virtually, with settings including private homes, congregate living facilities, and other home environments.
Integration of clinical practices, data dashboards, and operational processes that support company performance goals into daily workflows.
Promotion of and participation in patient engagement and experience initiatives.
Maintenance of comprehensive medical records including timely and accurate documentation and coding, medication reconciliation, and accurate recording of patient care activities.
Active participation in care team huddles, operational huddles, educational training, and clinical partnership meetings as requested.
Collaboration with external providers and clinical partners, including outreach and ongoing communication to coordinate care and sustain key partnerships with PCPs and other clinical partners.
Participation in after-hours on-call schedule and other clinical care cross-coverage as requested.
Other market-specific duties as assigned.
Qualifications
Licensed Nurse Practitioner with ANCC or AANP certification, or board-certified Physician Assistant (PMHNP or NNP only not eligible).
Active licensure in the state of California. 103 NP licensure is a plus.
Active DEA licensure and state-specific controlled substance registration, as applicable.
Minimum of 3 years experience; in-home/virtual experience is preferred.
Advanced training in geriatrics and/or palliative medicine is preferred.
Certification in basic life support (BLS) is required.
Clinical experience caring for aging adults and adults with complex care needs is required.
Experience caring for patients in home settings including private homes and congregate living facilities preferred.
Previous experience with managed care and/or value-based delivery is preferred.
Advanced computer and software skills are required.
Ability to travel as needed promptly to locations with limited public transportation; proof of liability and property damage insurance required.
Vehicle requirements: Licensed ≥5 years, no serious driving violations, no more than 2 moving violations or 1 at-fault accident in last 3 years.
Compensation Base salary range $160,000 to $170,000 plus annual bonus. Compensation may fall outside this range based on qualifications, skills, and experience.
