Fee for Service Psych Mental Health Rehabilitation Practitioner Service Unit Compensation:

$40.00 per service unit, or $42.00 per service unit for Bilingual Care Managers Training Compensation:

$30.00 per hour for all required training Hours:

Immediate The Association for Mental Health and Wellness is seeking a passionate Fee for Psych Mental Health Rehabilitation Practitioner in our CORE program to make a difference in the lives of adults who have experienced trauma, significant behavioral/physical health and/or substance abuse challenges. The Fee for Service Psych Mental Health Rehabilitation Practitioner total compensation per service unit includes travel time, required administrative tasks, and the anticipated time to complete the client services. Service delivery is community based throughout Suffolk County. Key responsibilities will include, but are not limited to Identify strategies aimed at minimizing the negative effects of symptoms of mental illness Co-creation of person-centered individualized Care Plan based upon the goals and objectives identified by the client through assessment Provide support towards goal acquisition Assess needs related to daily living skills, social skills Link individuals to community resources in order to support community integration and connectedness. Promote and assist with advocacy regarding entitlement benefits such as food, clothing, and shelter. Assist individuals effectively respond to and recognize triggers that would exacerbate symptoms.

Qualifications:

Licensed Mental Health Professional (LMSW, LCSW. LMHC, LCAT) preferred Bi-lingual skills and individuals with personal recovery experience are highly encouraged to apply. Excellent inter-personal communication skills including the ability to establish empathic, empowering relationships with people who have experienced psychiatric and substance abuse disabilities and trauma as well as debilitating physical conditions. Strong communication skills with health professionals is also required. Ability to provide cultural and linguistically appropriate services. An understanding of the key elements of recovery and wellness, including the role of family and self-help. Basic knowledge of the public health/mental health system, public entitlement and social service benefits. Excellent organizational, time management and computer/technology skills such as Microsoft Word, Excel, and Outlook (agency uses electronic health records and multiple IT platforms). Knowledge of Health Homes, HARP and SPA housing. Candidate must be a reliable self-starter with the ability to work both independently and as part of a team. Clean and Valid NYS Driver’s License.
