Position: Bi-Lingual Children’s Outreach Care Manager Compensation: $45,000K – 47,000 per year; plus competitive benefits packages Location: Ronkonkoma, NY– travel throughout Suffolk County Hours: 37.5 hours per week Start Date: Immediately The Children’s Outreach Care Manager connect with the local community and establishes supportive helping relationships with children and their families who have significant behavioral and/or physical health conditions. The Outreach Care Manager is responsible for the initial consent and enrollment process, collaborative data entry. The Outreach Care Manager primarily works in the community, with travel throughout Suffolk County as a key component of the role

While agency vehicles are available, candidates must have a reliable personal vehicle and a valid, clean NYS Driver’s License. Mileage reimbursement is provided. Outreach Care Managers operate within collaborative teams, receive weekly supervision, and benefit from ongoing support. Laptops and cell phones are provided to support field-based work. Duties/Responsibilities: Eligibility screening, referral triage outreach and engagement Meeting with youth and family for initial consent and enrollment process Contact providers to obtain backup eligibility documentation Reviews consents for accuracy Provide comprehensive care management Attend community outreach events Enters notes and uploads consents to EHR
Qualifications:
Required Skills/Abilities: Ability to pay close attention to detail to ensure accuracy in the consent and enrollment process Excellent inter-personal communication including the ability to establish empathic, empowering relationships with children and families who have experienced behavioral health issues, substance use, complex trauma as well as physical conditions which impact their functioning. Strong collaborative communication/documentation skills with health/mental health professionals, educators, child welfare services and involved in the care. Understanding of the key elements of recovery and wellness, including the role of family and self-help; Sound organization skills including computer skills (agency uses electronic health records). Basic knowledge of the children’s public health/mental health system, child welfare, public entitlement and social service programs; A reliable self-starter with the ability to work both independently and as part of an interdisciplinary team. Candidate must be flexible in availability to work evenings, after school, weekends in order to meet with employed caregivers. Strong writing and computer skills, as electronic health record is used for documentation. Basic knowledge in computer applications such as Microsoft Word, Excel, Outlook and Social Media. Clean NY State Driver’s License as some driving with clients will be necessary. Must have reliable transportation for this position. Bi-lingual, English/Spanish required; stipend awarded for assessed and verified bi-lingual proficiency. Education and Experience: A Bachelor’s degree in a human service/health discipline and two years’ experience working with children is required. Candidates with Master’s degrees related to physical/mental health are also encouraged to apply. Current or previous certification in completing Child and Adolescent Needs and Strengths Comprehensive Multisystem Assessments (CANS) a plus.
