Overview

LVAD (Left Ventricular Assist Device) Nurse Coordinator- Heart & Vascular. Full Time. The LVAD Program Nurse Coordinator- Heart & Vascular will play a vital role in enhancing the quality of patient care and optimizing clinical practices within our organization. Reporting to the service line director, you will be responsible for coordinating and leading the planning and implementation of clinical initiatives to improve the quality, efficiency, and cost-effectiveness of care delivery for Left Ventricular Assist Device. Responsibilities

Collaborate closely with a multidisciplinary team and identified physician leaders to optimize patient outcomes and promote evidence-based practices. Work with the heart failure and structural heart teams to enhance care coordination and ensure that appropriate patients receive timely procedures. Strive to provide timely access to treatments, reduce readmissions, improve clinical outcomes, enhance patient satisfaction, and effectively manage financial resources. Coordinate care for patients primarily being evaluated for cardiovascular procedures. Partner with interventional cardiologists, the cardiothoracic surgical team, and clinical staff across inpatient and outpatient settings within Baystate Health and external facilities to coordinate pre- and post-procedure care for cardiovascular patients. Lead front-line nursing coordination with excellent leadership, communication, and interpersonal skills; apply expert clinical knowledge to collect and assess data with the multidisciplinary team and the ACC-NCDR Data Coordinator to improve quality outcomes. Collaborate with inter-professional team members to enhance outcomes across the continuum of care by utilizing, critiquing, and/or conducting research to promote evidence-based practice. Evaluate current clinical practices of referring providers and staff and provide education to ensure research-based best practice is delivered; participate in development, execution, and evaluation of the Heart & Vascular program. Communicate professionally for programmatic changes; use tact, discretion, diplomacy, and problem-solving to support team development and provide feedback. Assist patients and families in understanding and navigating complex clinical evaluations; maintain expert clinical knowledge and advanced nursing certifications in the specialty to serve as a strong resource. Required Experience

Registered RN with a BSN required (masters preferred). Minimum of 5 years acute care experience including critical care. Clinical knowledge of disease/illness/treatment options specific to the specialty. Excellent communication and organizational skills. Ability to lead groups with varying opinions and provide constructive feedback. Ability to deliver presentations and facilitate large group meetings. Well developed PC skills including MS Office: Word, PowerPoint, Excel. Hours & Location
