ABL Health Care is a trusted provider of home healthcare and hospice services committed to delivering compassionate, personalized care. We are seeking a compassionate, organized, and team-oriented Physical Therapist (PT) to join our Home Health team in Half Moon Bay, CA.
Position Overview As a Physical Therapist (PT), you will evaluate patients’ physical health, develop individualized treatment plans, and work to improve mobility, strength, and overall quality of life. You will collaborate closely with our Clinical Director and Clinical Staff, ensuring the highest standards of care while providing support both remotely and independently in the field.

Responsibilities
Provide physical therapy services within the scope of care, following the patient’s individualized plan, in coordination with the interdisciplinary team
Administer treatments, monitor patient conditions, and report signs/symptoms indicating a change in patient condition to Physician, Case Manager, and/or Director of Nursing as needed
Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device
Comply with all company policies and procedures to ensure high standards of care
Requirements
Current Physical Therapist (PT) license to practice in California – required
Experience in Home Health or Hospice care – 1 year required
Current California driver’s license
Able to travel within a 15–25 mile radius on a regular basis
