Overview
Trustbridge Hospice, a part of Empath Health is seeking a detail-oriented, process-focused Quality Improvement Specialist (Registered Nurse or Licensed Clinical Social Worker) in West Palm Beach, FL to help advance our commitment to exceptional care through data-driven insights, collaboration, and continuous improvement. This role plays a vital part in ensuring our programs and services meet regulatory requirements, exceed quality standards, and reflect best practices that benefit our patients, families, and community. The ideal candidate brings strong analytical skills, a deep understanding of Hospice and Palliative Care quality measures, and the ability to collaborate across departments. Experience with auditing, data analysis, creating powerful presentations and ability to facilitate performance improvement projects. Monitor, analyze, and report Hospice and Palliative Care quality measures to identify trends, gaps, and opportunities for improvement.
Conduct audits, compliance reviews, and performance improvement projects in collaboration with internal teams.
Support the development, implementation, and tracking of corrective action plans to ensure sustainable improvements.
Maintain knowledge of current regulations and standards, ensuring organizational compliance with federal, state, and accrediting bodies.
Collaborate with clinical and operational teams to promote a culture of safety, quality, and continuous learning.

What You’ll Need
Must be a Registered Nurse OR Licensed Clinical Social Worker
Knowledge of Hospice and Palliative care processes and quality measures
2–4 years of experience in healthcare quality improvement, medical, compliance, or a related discipline.
Knowledge of quality improvement methodologies.
Proficiency in data analysis and reporting; experience with quality management software a plus.
Strong organizational, communication, and problem-solving skills.
