Trustbridge Hospice , part of Empath Health, seeks a compassionate and customer‑focused Admissions Coordinator 2.
This full‑time position works Monday through Thursday, 7:30 a.m. – 6:00 p.m., supporting the admissions process and ensuring a seamless, empathetic experience for patients and families during one of life’s most meaningful transitions.
Since 1978, Trustbridge Hospice has cared for more than 200,000 South Florida families. As a community‑based nonprofit we provide 24/7 hospice and palliative care, caregiver support, and bereavement services for families facing serious illness.
What You’ll Do
Facilitate the conversion of referrals to admissions through completion of the intake process.
Provide exceptional customer service to patients, families, and referral sources with empathy and professionalism.
Communicate effectively with interdisciplinary team members to ensure timely and accurate admissions.
Maintain accurate records and documentation in accordance with organizational policies.
Support a compassionate, patient‑centered environment that reflects the mission and values of Trustbridge Hospice.

What You’ll Need
Associate degree preferred.
Three (3) years of experience in healthcare administration or customer service.
Knowledge of hospice or home‑health programs preferred, with willingness to develop clinical understanding.
Ability to handle a high call volume.
Strong communication, organization, and computer skills.
Bilingual (English/Spanish) preferred.
