Gurwin Healthcare System provides the following base pay range:
$103,350.00/yr - $107,250.00/yr .
We are seeking an RN Clinical Manager to lead the Licensed Home Care Division, ensuring high quality clinical services and compliance with state regulations.
Duties and Responsibilities
Assume a leadership role relative to the clinical aspects of the LHCSA, overseeing and directing all clinical issues, HHA training compliance, and successfully meeting all NYSDOH clinical requirements.
Competently address family and patient complaints and issues; work effectively with referral sources.
Assume total clinical responsibility for active census, ensuring all visits are conducted timely and handling emergencies as needed; full responsibility for clinical on‑call.
Conduct quarterly Professional Advisory Committee meetings, creating and trending required outcomes and data.
Ensure that a complete assessment and coordinated plan of care is developed and adhered to for all patients referred to the LHCSA; work effectively with referring facilities to ensure cohesive transitional care.
Participate in formulating, evaluating, and revising agency policies and procedures.
Oversee employee in‑service and orientation to ensure staff compliance with NYS DOH requirements; full responsibility for the oversight of the HHATP.
Support office staff to ensure compliance with all NYSDOH requirements (e.g., completing aide supervision visits, aide competencies, reviewing and approving HHA medicals).
Assume a leadership role in Emergency Preparedness through participation in DOH drills and activations.
Successfully meet targeted performance indicators related to oversight and management of clinical staff.
Maintain EMR patient files (current and discharged).
Demonstrate cooperation and professionalism with peers and supervisors.
Perform job functions with maximum efficiency and effectiveness.
Accept constructive feedback, comprehend overall concepts communicated, and make necessary changes to improve results.
Consistently focus on customer service; identify and resolve issues to full recovery.
Maintain confidentiality consistent with HIPAA regulations; adhere to compliance policies and procedures.
Perform related tasks as assigned.
Qualifications
Current New York State Nursing license and BSN, or a bachelor’s degree in a related health field or equivalent experience.
Minimum of four years’ experience, two of which in a supervisory capacity overseeing direct nursing services.
Excellent educator, home health aide assessment and patient assessment skills; ability to set priorities.
