Physical Therapist (PT) Home Health – Full Time or Part Time
ABL Health Care, Santa Clara, CA – Join our Home Health team as a full‑time or part‑time Physical Therapist.
Position Overview: Experienced, self‑driven PT who evaluates patients, develops individualized treatment plans, and improves mobility, strength, and overall quality of life. Collaborates closely with Clinical Director and interdisciplinary team, providing support remotely and independently in the field.

Responsibilities
Provide physical therapy services within scope of care, following patient‑specific plan.
Administer treatments, monitor patient conditions, and report changes to Physician, Case Manager, or Director of Nursing.
Maintain accurate clinical records and submit documentation per company policy using company devices.
Comply with all company policies and procedures to ensure high standards of care.
Requirements
Current PT license to practice in California.
At least 1 year of Home Health or Hospice care experience.
Current California Driver’s License.
Able to travel within a 15‑25 mile radius regularly.
