Case Manager, Homelessness Prevention Initiative (HPI) (HomeBase-Stuyvesant Place) Who We Are:

We offer more than 180 integrated programs in Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services. CAMBA serves a diverse cross‑section of New Yorkers, with over 85% of families living in poverty.

HomeBase

is a CAMBA program providing case management and essential services to at-risk individuals and families in Brooklyn and Staten Island to prevent homelessness.

What the Case Manager Does

Prescreen clients over the telephone for eligibility and schedule intake appointments.

Conduct initial intake or assessment of clients and families and periodic reassessments.

Validate documentation establishing client eligibility and make file copies.

Assist clients in completing all CAMBA intake applications and forms.

Refer clients to appropriate services within and outside CAMBA.

Follow up with clients and referral organizations regarding contact and progress.

Evaluate actual living conditions of clients through home visits or inspections.

Prepare marketing materials for the program.

Enter client data and progress information into the automated database.

Reach out and market the program to recruit clients.

Create and maintain client files, both hard copy and electronic.

Help clients break through barriers, advocate for themselves, and move toward self‑sufficiency.

Recommend strategies to persuade clients to participate more fully in the process.

Monitor clients’ progress toward goals via scheduled telephone contact or visits, documenting via progress notes.

Recommend closing of cases when clients have achieved goals and maintained stability, or are lost‑to‑service, or become ineligible.

Meet daily/weekly/monthly goals and tasks as assigned and funder guidelines.

Complete and submit subsidy applications and requests for financial assistance.

Provide required information for weekly/monthly/quarterly/annual reports.

Education, Experience, Licenses, and/or Certifications Required

Associate’s degree or four years of relevant experience.

Bachelor’s degree or higher.

Preferred Qualifications

Bi‑lingual in English and Spanish.

Compensation $48,410‑$52,530 annually.

Benefits Full‑time (35 hr per week) with a comprehensive benefits package including health, dental, 403(b) retirement plan with employer match, paid time off, and paid holidays.
