Primary Care Physician (PCP) Profile

Accountability for outcomes : Demonstrate strong clinical care and cost‑effectiveness for a panel of up to 450 patients. Build trusting relationships and influence patient behavior to improve outcomes.

Coaching for health : Act as a health coach, setting short‑ and long‑term goals with patients and following up regularly to drive improvement.

Simplifying for action : Prioritize and simplify care to make behavior change actionable for patients and for the care team.

Essential Job Duties / Responsibilities

Provide independent care for patients with acute and chronic illnesses in older adults.

Take full accountability for patient care and outcomes, seeking specialist consultation when needed while remaining engaged.

Engage with hospitalist teams for patients admitted to hospitals.

Assess, diagnose, treat, manage, educate, promote health, coordinate care, and document for patients with acute and complex chronic needs.

Lead the care team—care promoter, facilitator, and coordinator—for patients able to come to the office.

When patients are unable to attend the office, collaborate with the transitional care team, case managers, and other local resources.

Participate in center management, cover for other providers, and help recruit patients and providers.

Supervise and collaborate with an Advanced Practice Practitioner (APP), remaining accountable for the APP’s actions.

Perform other duties as assigned and modified at the manager’s discretion.

Availability and accessibility to build trust—open for walk‑in visits and responsive to calls and messages.

Evidence‑based medicine—stay current on evidence‑based practice, including lifestyle and nutrition interventions.

Physician leadership—continually develop leadership skills for patient, team, and center benefit.

Focus on quality—work with teams to achieve high quality outcomes and continuous improvement.

Influence—compassionately guide patients, teams, and self toward best outcomes.

Self‑care—maintain physical, mental, social, and spiritual well‑being for sustainable practice.

Proficient in Microsoft Office Suite and related software.

Travel willingness—locally, regionally, and nationally as needed for training or recruiting.

Fluency in English (verbal and written). Other language fluency may be required.

Independent judgment and flexibility to work evenings, weekends, and holidays.

Education and Experience Criteria

MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.

Active State Medical License in the relevant state(s) of employment.

Board certification or eligibility in the specialty; ongoing maintenance of certification (MOC, CME, or re‑examination).

Valid DEA number for Schedule II–V controlled substances.

Basic Life Support (BLS) certification within 90 days of employment.

Pay Range $214,700 – $306,714.
