Key Responsibilities
Provide independent clinical care for acute and chronic illnesses in older adults, managing diagnoses, treatment, education, health promotion, and documentation.
Take full accountability for patient outcomes, seeking specialist consultation when necessary.
Engage with hospitalists and transitional care teams for patients in hospitals, SNFs, LTCs, or homebound settings.
Lead a care team consisting of a medical assistant, care facilitator, and care coordinator for office‑based patients.
Assist with center management, cover for other providers, and support recruitment of patients and additional providers.
Collaborate with an Advanced Practice Practitioner (APP), remaining accountable for the APP’s actions while employed with the company.
Perform additional duties as assigned by management.
Competencies for Success
Availability and accessibility to build patient trust, including walk‑in visits and timely response to calls and messages.
Service orientation, providing care just as one would for a family member.
Evidence‑based medicine and staying current on lifestyle and nutritional interventions.
Continuous leadership development for patient, team, center, and company benefit.
Commitment to high‑quality care and systematic improvement of practice operations.
Ability to influence patients, teams, and oneself compassionately toward optimal outcomes.
Self‑care for sustained physical, mental, social, and spiritual health.
Proficiency in Microsoft Office Suite and related software.
Willingness to travel locally and occasionally nationally for training or recruitment.
Fluency in English; fluency in other languages may be required for certain centers.
Independent judgment in patient care and practice management.
Education and Experience
MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.
State Medical License (current and active) for the state(s) of employment.
Board certification (preferably) or board eligibility in the specialty; must maintain certification via MOC or CME.
Current DEA number for Schedule II–V controlled substances.
Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within the first 90 days of employment.
Pay Range $221,141 – $315,915 Salary
