Responsibilities As an RN in the Case Management department, you will collaborate to enhance the delivery of patient care services across the continuum of care.

You will evaluate, predict, and facilitate the trajectory of patient care, employing a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation to meet individual health‑care needs.

Your work will help ensure high‑quality patient care, reduce costs, and promote reimbursement.

You will be supported by strong leaders, subject‑matter experts, and case‑management resource specialists.

The department offers in‑depth orientation, onboarding, an innovative “Novice to Expert” program, and a Clinical Recognition Program to formally recognize clinical staff.

Qualifications

ADN required, BSN preferred

MA Registered Nurse license required

Licenses & Credentials MA Registered Nurse required.

Pay Range: $41.72 – $105.65 per hour
