Schedule Notes: 10 PM - 8 AM and 3:30 PM - 11:30
ADDITIONAL SKILLS REQUIRED CHEMO PREP FLOATING: CRITICAL CARE, PEDS, ONCOLOGY, TRANSPLANT, MEDICINE, CARDIAC
Duties and Responsibilities: Key Responsibilities Medication Preparation & Dispensing Accurately and efficiently prepares, labels, and dispenses medications in accordance with established policies, procedures, and regulatory standards. Resolves questions related to formulary and non-formulary medications, ensuring compliance with institutional guidelines. Clinical Support & Therapeutic Recommendations Provides evidence-based dosing and therapeutic recommendations, considering patient age, renal/hepatic function, and pharmacokinetic principles. Monitors patient drug therapy for effectiveness, safety, potential interactions, and appropriateness of continued treatment. Patient & Staff Education Educates patients on medication use, including dosing, schedule adherence, side effects, storage, and interactions with other drugs or supplements. Offers pharmacological information and consultation to physicians, nurses, and other healthcare team members.
Skills & Competencies Accurate and timely dispensing of medications Strong knowledge of formulary management Ability to interpret and apply pharmacokinetic and age-based dosing principles Effective patient and staff communication and education Strong clinical judgment and problem-solving skills
Education & Experience Graduate of an ACPE-accredited College of Pharmacy 1–2 years of experience as a licensed Pharmacist
