Cobble Hill Health Center is a not-for-profit long- and short-term care facility providing services to the disabled and elderly for over 40 years. The MDS Assessor will play an integral role in helping us to achieve our vision: to consistently exceed expectations and set the standard for excellence in residential and community care.

Responsibilities

Lead and oversee all aspects of the MDS process, ensuring compliance, accuracy, and optimal reimbursement.

Monitor CMI, lead related meetings, and regularly communicate Quality Measures to the interdisciplinary team.

Review MDS assessments to ensure accurate coding, conduct PDPM reviews upon admission and prior to submission.

Collaborate with rehab to verify correct primary diagnoses.

Review UB04s, manage ADR responses and appeals, coordinate with Livanta, and submit documentation to the billing company.

Participate in audit responses, run Medicare Part B reports, and actively participate in key facility meetings including Medical Board, Therapy, QAPI, and Utilization Review.

Ensure and maximize proper reimbursement while maintaining compliance with federal and state guidelines.

Qualifications

Current New York State RN license.

Three or more years of experience in a long-term care facility as an MDS Coordinator.

Knowledge of Medicare and Medicaid criteria and assessment regulations.

Excellent listening and communication skills.

Ability to complete documentation in a timely manner, with attention to detail and accuracy.

RAC Certification and prior leadership experience are strongly preferred.
