Overview
Adventist HealthCare Home Health is hiring a Physical Therapist in the Montgomery County Territory. Eligible candidates will receive up to a $12,000 sign‑on bonus. Current AHC employees are not eligible for the bonus.
Responsibilities
Perform initial examinations and evaluations to establish impairment, diagnosis, prognosis, and plan of care for all patients in acute, outpatient, or inpatient settings.
Provide or direct and supervise therapy interventions consistent with the examination, evaluation, diagnosis, and plan of care. Engage patients, families, and team members in planning, implementation, and assessment.
Reexamine patients as necessary during an episode of care to evaluate progress or change in status and modify the plan of care accordingly.
Plan for discharge by considering achievement of anticipated goals and expected outcomes, and provide appropriate follow‑up or referral. Discontinue intervention when patients no longer benefit from therapy.
Communicate, coordinate, and document all aspects of patient management, including exam results, diagnosis, prognosis, plan of care, interventions, response to interventions, barriers to progress, reexamination, and discharge/discontinuation, with members of the care team.
Supervise and direct the patient‑care activities of assigned rehab aides, office assistants, and volunteers.
Manage schedule to accommodate census and patient load and inform management of any issues with caseload management.
Attend team meetings and effectively communicate patient status, using the medical record or other treating therapists as indicated.
Maintain individual professional development activities.
Qualifications
Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited program.
1‑year site‑specific experience preferred.
Active Maryland License.
Valid driver’s license with agency insurance requirements.
Active American Heart Association Basic Life Support (BLS) certification required.
Work Schedule Full‑time Monday‑Friday with one weekend day per month and holiday rotation.
