Ambulatory Clinical Pharmacist, Multi‑Specialty Float Pharmacists are integral members of the health care team at UC Davis Health. They share responsibility for comprehensive medication management, distribution of medications to inpatients and ambulatory patients, teaching of pharmacy students, residents and staff, clinical research, and participation in Department and Hospital Committees, taskforces and key initiatives related to pharmaceutical care. This position works in the Ambulatory Clinics and Specialty Pharmacy Hub to provide services to patients, helping patients obtain their medications and monitor drug therapy to achieve desired outcomes, avoid drug toxicity, and provide dosing of medication based on patient demographics, guidelines and response.

Minimum Qualifications

California Pharmacist License eligible and possess a valid license at the time of hire

American Heart Association (AHA) or American Red Cross Basic Life Support (BLS), or UC Davis Health CPR at the time of hire

Doctor of Pharmacy Degree from an accredited college or university, or equivalent training

Completion of a Post Graduate Year One (PGY‑1) Pharmacy Practice residency, or 3 years of relevant pharmacist experience

Experience with navigating third‑party reimbursement programs

Experience precepting and evaluating residents and students

Thorough understanding and competency with aseptic technique and safe handling of hazardous meds

Computer skills that facilitate clinical patient data medication order entry and other basic clinical software programs

Demonstrated ability to effectively communicate with and educate patients about their medication therapy

Written and oral communication skills to clearly and concisely provide information, explanations and instructions to, and to elicit information from others with varying levels of ability to understand, including individuals who are ill or distressed, or who may not speak English

Knowledge of current ambulatory care guidelines (i.e. NCEP, JNC, ADA)

Demonstrated ability to identify medication‑related problems in patients, communicate them to a patient's physician and offer recommendations

Complete working knowledge of, and skill to apply, practices, principles and techniques of patient care related to area of expertise

Decision‑making skills to independently exercise delegated authority to take action, and to recognize when it is appropriate to seek assistance from others when limits of delegated authority have been reached

Preferred Qualifications

Board of Pharmacy Specialties (BPS) or Certified Specialty Pharmacist (CSP) Certification

Completion of a Post Graduate Year Two (PGY‑2) residency

Problem‑solving skills to independently recognize and define a problem, identify the resources available to help solve the problem, create and implement viable solutions, and follow through to ensure the problem is resolved to the satisfaction of all parties.

Ability to lead the work of technicians and establish priorities of workflow

The largest pharmacy residency program in the country has 46 residents in 13 different programs. Pharmacy services encompass inpatient pharmacy services and a comprehensive footprint in the ambulatory care space, with clinical pharmacists embedded within our clinics and specialty care locations.

Work Environment UC Davis is a smoke and tobacco free campus effective January 1, 2014. The position includes day, evening, night and rotating weekend shifts. TB skin testing required annually. Must have good personal hygiene and dress in a professional manner consistent with the Departmental professional behavior policy. Abide by Departmental attendance standards. Special protective equipment must be used for required preparation of hazardous substances and spill cleanup.
