Long-Term Care Nurse
Location:

Shift Schedule:
3:30PM - Midnight or NIGHTS
Position Description:
The Long-Term Care Registered Nurse (RN) delivers comprehensive nursing care to residents across three long-term care units, including two 45-bed units for long-term and short-stay residents, and one 10-bed unit providing hospice and palliative care services. The RN provides individualized, age-specific care that focuses on enhancing quality of life, comfort, and residents’ health needs throughout their stay. This role requires strong clinical skills, compassionate care delivery, and effective communication to meet the unique needs of long-term care residents and their families in a supportive, patient-centered environment.
Responsibilities
Unit Description:
Three long-term care units: two 45-bed units (long-term and short-stay residents) and one 10-bed unit for hospice and palliative care.
Care Delivery:
Assess, plan, implement, and evaluate care based on age-specific components; coordinate care across the continuum; provide family education; and perform nursing care following established policies and guidelines.
Nursing Duties:
Manage ADLs, assessments, BCMA medication administration, and procedures; perform barcode medication administration; IV therapy, blood transfusions, vital signs, hydration, and tube feeding; manage behavioral patients; participate in interdisciplinary teams; handle admissions, discharges, and transfers; delegate to ancillary staff; document in electronic medical records; provide patient transport and emergency response as needed.
Minimum Qualifications
Requirements:
BLS certification and at least three (3) years of experience in acute care or other relevant areas.
The personnel assigned shall possess an unrestricted license to practice medicine in one of the States, Territories, or the District of Columbia.
