Primary Care Physician – ChenMed

The PCP independently provides care for patients with acute and chronic illnesses among older adults. The PCP takes full accountability for patient care and outcomes, seeks specialist consultation when needed, and remains responsible for detailed patient care. The PCP engages with hospitalists whenever a patient is hospitalized and coordinates with care teams for non‑office patients (hospital, SNF, LTC, homebound). The PCP leads a care team including medical assistant, care facilitator, and care coordinator for office patients. The PCP collaborates with an APP, remains accountable for the APP’s actions, and assists with recruiting patients and providers. Participates in risk and quality management programs, clinical meetings, and other relevant meetings to promote patient health and organizational goals. Performs other duties as assigned by management. Knowledge, Skills & Abilities
Availability and accessibility for patients, including walk‑in visits and timely communication. Service orientation: provides care that one would want for a family member. Evidence‑based medicine and staying updated on clinical, nutritional, and lifestyle interventions. Leadership skills to support patients, teams, center, and company. Commitment to quality: continuous improvement of practice and center operations. Influence: compassionate influence on patients and teams. Self‑care: maintains physical, mental, social, and spiritual well‑being. Proficiency with Microsoft Office Suite and related software. Willingness to travel locally and occasionally regionally/nationally for training and recruiting. Fluency in English (verbal & written); other language fluency required where indicated. Independent judgment in daily clinical and operational decisions. Pay Range
