Overview ChenMed is a physician‑led, mission‑driven primary care organization that is one of the most successful full‑risk Medicare Advantage providers in the nation. We are rapidly expanding and need dedicated primary care providers who are passionate about honoring seniors with affordable VIP care that delivers better health.
Essential Job Duties and Responsibilities
The PCP independently provides care for patients with acute and chronic illnesses encountered in older adult patients.
The PCP takes full accountability for patient care and outcomes and will appropriately seek consultation from specialists when needed, while remaining involved in the detailed care of the patient.
The PCP engages with hospitalists whenever one of their patients is in the hospital (regardless of whether the hospitalist works for ChenMed).
The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination and documentation for patients with acute and complex chronic health needs.
The PCP leads a care team consisting of a care promoter (medical assistant), a care facilitator and a care coordinator for patients able to come to the office.
For patients unable to come to the office— in hospital, SNF, LTC or homebound—the PCP engages with the transitional care team and other resources that may be available depending on the market.
The PCP has an active role in the management of their center and will help cover for other providers who may be out for various reasons. It is also expected that each PCP will take an active role as needed in recruiting patients for the center and additional providers for the company.
The PCP supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remains accountable for the actions of the APP while employed with the company.
Performs other duties as assigned and modified at manager’s discretion.

Availability and accessibility to build trust— PCPs will be open and available for walk‑in visits and answer phone calls and messages in a timely manner.
Service orientation— PCPs provide care they would want for a family member or themselves to each patient at every interaction.
Evidence‑based medicine— PCP remains updated on evidence‑based medicine and recognizes lifestyle and nutrition play large roles in patient outcomes.
Physician leadership— PCP continually works to develop and improve leadership skills for the benefit of patients, teams, center and company.
Quality— PCPs work with care teams toward high quality outcomes and continually seek systematic improvements.
Influence— PCPs competently and compassionately influence patients, teams and themselves to achieve the best outcomes.
Self‑care— PCPs take care of themselves physically, mentally, socially and spiritually to sustain physician wellness.
Proficiency in Microsoft Office Suite and other office software.
Willingness to travel locally as needed, and nationally for initial training (2 weeks) and occasionally regionally and nationally for recruiting or training purposes.
Fluency in English, verbal and written. Fluency in other languages may be required for some centers.
The job requires use and exercise of independent judgment.
Pay Range $214,700 - $306,714 Salary.
