Base Salary + Performance Incentives (equivalent to $45 per hour)
About the Role April Parker Foundation is a nonprofit changing community health and housing. As a care manager, you will work directly with Medi-Cal members who have complex medical, behavioral, and social needs, helping them navigate care, access community resources, and achieve stability.
What You’ll Do
Conduct outreach, assessments, and individualized care plans for ECM and CS members.
Coordinate medical, behavioral, and social‑service supports, including housing navigation and tenancy services.
Complete timely documentation and progress tracking in APF systems.
Provide in‑person, telephonic, and virtual encounters based on member needs.
Collaborate with health plans, community partners, and APF multidisciplinary teams.
Maintain strict confidentiality and comply with Medi‑Cal, DHCS, and ILS guidelines.
Compensation Details Base Salary:
Each member equates to 2 paid hours per month (PMPM) at $25.00/hour ($50 PMPM); salary increases with caseload.
Incentive Pay:
$40 per member per month for delivery qualified, on top of base salary. Total compensation can reach $90 PMPM, equivalent to $45/hour.
Reimbursements & Stipends:
Mileage reimbursement at IRS rate
$50/month phone stipend
Reimbursement for approved work‑related expenses
Schedule:
Flexible, self‑directed hours provided care managers meet service delivery requirements and member availability.
Qualifications
Minimum 2 years of experience in case management, care coordination, or related field.
Knowledge of Medi‑Cal CalAIM programs, community resources, and social determinants of health.
Excellent documentation, organization, and communication skills.
Valid California Driver’s License, auto insurance, and reliable transportation.
Preferred: Bachelor’s degree or CHW certification; CA licensure (LCSW, LMFT, LPCC, RN, etc.).
