Position Summary We are a premier home health provider in our community, enhancing care experience for the patients, families and referral sources. As a Home Health RN, you will develop a plan of care in collaboration with the patient, family/caregiver, and physician orders, monitor the patient's conditions, illnesses or injuries, and provide ongoing nursing visits.
Responsibilities
Perform comprehensive assessments in the patient's homes and identify patient's needs.
Develop a care plan in collaboration with patient, caregivers, healthcare provider, case manager, and/or supervisor, and document appropriately.
Assess need for additional services and equipment on admission and on an ongoing basis, informing the case manager and/or supervisor promptly.
Assess the home environment and recommend improvements to enhance patient care, promote safety, and prevent adverse effects.
Take verbal orders from the healthcare provider as necessary promptly and document appropriately.
Initiate preventative, rehabilitative, therapeutic, comfort, and pain interventions and provide instructions to patients, caregivers, and staff promptly.
Maintain high standards of clinical care, stay current on medication issues and clinical updates.
Update patients' clinical records, including medication profiles, plan of care, OASIS, case conference, progress notes, and re-certification documentation as required.
Serve as a preceptor when needed.
Perform other duties as assigned.

Qualifications
Graduation from an accredited nursing program.
Current state license as a Registered Nurse and CPR certification.
At least one year of clinical practical nursing experience outside of school/rotations (Home Health experience preferred but not required).
Must have a vehicle, current driver's license and automobile insurance.
Interpersonal and communication skills necessary.
Ability to make home visits and provide skilled nursing assessments and care in the home.
Home Health and Home Care Home Base (HCHB) documentation experience preferred but not required.
