Role Description
This is a full-time on-site role for a Registered Nurse based in Marin County, CA. The Registered Nurse will provide patient-centered care and perform clinical assessments for individuals in their homes. Responsibilities include administering medications, monitoring patient progress, maintaining accurate records, and collaborating with an interdisciplinary healthcare team. The role involves ensuring adherence to care plans and educating patients and their families to promote overall health and wellness.
Responsibilities
Perform comprehensive assessments in the patients' homes and identifies patient's needs.
Develop a care plan in collaboration with patient/CGs, healthcare provider, case manager, and/or supervisor as assigned and document appropriately.
Assess the need for additional services and equipment on admission on an ongoing basis and inform the case manager and/or supervisor on a timely basis.
Assess the home environment and make recommendations that will enhance patient care, promote safety, and prevent adverse effects.
Take verbal orders from the healthcare provider as necessary promptly and documents appropriately.
Initiate appropriate preventative, rehabilitative, therapeutic, comfort, and pain interventions and provide instructions to patients/CGs and staff promptly.
Demonstrate professionalism by maintaining high standards of clinical care and staying current on clinical and medication issues.
Updates to patients' clinical records including medication profiles, plan of care, OASIS, case conference, progress notes, and re-certification documentation as required.
Serve as a preceptor when needed.
Qualifications
Graduation from an accredited nursing program.
Current state license as a Registered Nurse, and CPR certification.
At least one year of clinical practical nursing experience outside of school/rotations. Home Health experience preferred but not required.
Must have a vehicle and current driver's license and automobile insurance.
Interpersonal and communication skills necessary.
Ability to make home visits and provide skilled nursing assessments and care in the home.
Home Health and Home Care Home Base (HCHB) documentation experience preferred but not required.
