MDS Director
Lake Country Health Services, a leading post‑acute care provider in the Upper Midwest, is seeking a skilled MDS Director to oversee all aspects of the Resident Assessment Instrument (RAI) process and ensure accurate, compliant resident assessments.
Responsibilities
Lead the development and completion of the MDS for each resident in accordance with federal and state regulations.
Coordinate the creation of a comprehensive care plan and chair the interdisciplinary Team of Care.
Validate face‑validity of all MDS before electronic submission and ensure timely, accurate electronic entries.
Conduct monthly reviews of random MDS samples to confirm accuracy and consistency with the medical record.
Generate trigger assessments and develop resident protocols to initiate care plans.
Maintain MDS documentation and related clinical records in compliance with Medicare, Medicaid, OBRA, and other payor requirements.
Coordinate and assist with weekly interdisciplinary care conferences and Medicare meetings.
Provide training to nursing staff on clinical record documentation and MDS completion.
Ensure MDS submission and reporting processes (e.g., CASPER, validation reports) are accurate and timely.
Stay current with state and federal regulatory changes and ensure center compliance.
Maintain resident safety, rights, and HIPAA confidentiality in all documentation.
Qualifications – Required
Registered Nurse (RN) in good standing with the state licensing board.
Clinical experience and education in geriatrics with demonstrated competency in PPS assessment and OBRA compliance.
Excellent clinical assessment, organizational, and independent decision‑making skills.
Superior interpersonal communication and computer proficiency with Microsoft Office and MDS software.
Comprehensive knowledge of state and federal regulations governing post‑acute care.
Qualifications – Preferred
At least 2 years of post‑acute or health‑care experience.
Experience in a fast‑paced environment.
Background in post‑acute care or related healthcare settings.
