Position Overview The Family Nurse Practitioner provides leadership and supervision for evidence‑based treatment plans for Hepatitis C and co‑occurring disorders. The medical provider will deliver patient‑centered Hepatitis C prevention, diagnosis, and treatment, manage co‑occurring conditions, and coordinate care across interdisciplinary teams to provide whole‑person trauma‑informed care tailored to underserved populations. The role includes managing Hepatitis C cases, prescribing antiviral therapies, monitoring treatment outcomes, and addressing co‑occurring mental health and substance use disorders through behavioral health screenings, integrated care planning, and appropriate referral.
What You Will Do
Provide comprehensive whole‑person medical care services, focusing on diagnosis, treatment, and management of Hepatitis C and related health conditions, including mental health, psychiatric, and substance use disorders.
Conduct initial and ongoing health risk assessments, including screening for substance use, HIV, and other high‑risk behaviors or conditions.
For people who test positive for HCV RNA, provide on‑site direct‑acting antiviral‑based treatment, peer support, and care coordination.
Develop and document a plan of care with treatment goals and outcome evaluations in accordance with clinical standards.
Assess clients to determine who would benefit from in‑person or virtual Directly Observed Therapy (DOT) for HCV.
Coordinate care with internal teams and external partners, including medical, behavioral/mental health, pharmacy, obstetric providers, hospitals, and laboratories.
Follow up to ensure individuals are engaged and retained in care and treatment, and able to complete their HCV treatment.
Assist in navigation to prenatal care for chemically dependent pregnant patients through consultation and referral to obstetric providers.
Offer patient education on topics such as birth control, safer sex, and the health consequences of high‑risk behaviors.
Encourage appropriate family involvement in patient care, in alignment with patient preferences and care team recommendations.
Serve as a patient advocate and facilitate access to specialty and support services, helping patients navigate complex healthcare systems.
Collaborate with interdisciplinary team members, including medical providers, psychiatric providers, nurses, case managers, and outreach staff, to ensure whole‑person care.
Participate in mobile outreach in an outdoor environment and in clinic‑based services to increase access for underserved and high‑risk populations.
