About the Role Premier Health Group is seeking an experienced Quality Improvement Nurse (RN) to lead our Quality Assurance and Performance Improvement (QAPI) efforts across our home health programs. This role sits at the intersection of clinical excellence, regulatory compliance, and data‑driven quality improvement. The ideal candidate brings strong clinical judgment, deep knowledge of Medicare/Medicaid standards, and a passion for elevating care quality through analytics, technology, and continuous improvement.

Key Responsibilities Regulatory Compliance & Quality Oversight

Ensure agency‑wide compliance with Medicare, Medicaid, DC Health, and accreditation standards (CHAP, JCAHO).

Conduct ongoing reviews of clinical documentation, including OASIS, care plans, and monthly nursing notes.

Identify documentation gaps and collaborate with clinicians to implement corrective actions.

Validate accuracy of diagnoses and adherence to physician/APRN orders.

Lead internal audits, support external survey readiness, and oversee QAPI program execution.

Clinical Quality Review & Performance Improvement

Track and analyze quality indicators (infection rates, falls, readmissions, incidents, complaints, utilization metrics).

Trend and interpret clinical data to identify performance gaps and implement process improvements.

Conduct monthly/quarterly audits of active and discharged records.

Prepare written quality reports highlighting trends, findings, and recommendations.

Lead quarterly QAPI meetings and coordinate interdisciplinary improvement initiatives.

Staff Training, Education & Performance Support

Assist with onboarding of clinical staff, emphasizing compliance and accurate documentation.

Provide ongoing training and in‑services based on audit results and regulatory updates.

Deliver performance feedback to support clinical competency, professional growth, and evaluation processes.

Utilize EHR systems, analytics dashboards, and reporting tools to monitor quality metrics.

Collaborate with technology teams to integrate AI‑driven insights and documentation support tools.

Support digital transformation efforts to improve efficiency, accuracy, and compliance.

Qualifications

Certification in Healthcare Quality (CPHQ) or Healthcare Compliance (CHC) preferred.

Minimum 2 years of home health or long‑term care experience preferred.

Minimum 2 years of Quality Assurance or Performance Improvement experience.

Strong knowledge of Medicare/Medicaid home health regulations and OASIS requirements.

Proficiency in ICD‑10 coding, documentation standards, and clinical auditing.

Strong analytical, communication, and problem‑solving skills.

Comfortable using technology, data tools, and digital healthcare platforms.

Ability to collaborate effectively and lead quality‑driven initiatives.
