Registered Nurse, Client Care Coordinator – 815 Residential Services

Job Summary As a Client Care Coordinator, you will coordinate team‑based care, collaborate with clients, families, community resources, and physicians, and focus on coordination, communication, and collaboration to support clients toward optimal outcomes.
Key Responsibilities Leadership Responsibilities
Champion change and adapt to evolving systems.
Participate in performance and quality improvement initiatives.
Provide medical‑clinical leadership to the intake team.
Organizational Responsibilities
Assist the Nurse Director in accreditation readiness and ongoing quality monitoring.
Translate best‑practice standards into daily care processes.
Coordinate care across multidisciplinary teams, including MAT, primary care, behavioral health, psychiatry, and withdrawal management.
Nurse Care Coordinator Responsibilities
Educate clients on conditions, treatment options, and medication management.
Develop, implement, and monitor client‑specific care plans with physicians and other stakeholders.
Coordinate complex care for clients accessing internal and external health services.
Refer clients to appropriate medical services (primary care, psychiatry, dental, addiction services).
Share relevant client information with providers (medication lists, problem lists, discharge summaries).
Perform additional duties as assigned.
Qualifications Education, Certification, and Experience
Active registered nursing license in California.
BLS certification within 30 days of hire.
Experience with safety‑net populations and substance‑use disorders.
Experience addressing mental‑health, criminal‑background, and socioeconomic barriers.
Desired Qualifications
Master’s degree in Case Management.
Previous experience in hospital acute care, community health centers, prior authorization, utilization review/management.
Experience with Welligent and/or EPIC EHR.
Knowledge and Skills
Knowledge of electronic medical records.
Knowledge of co‑occurring disorders and trauma‑informed treatment.
Knowledge of clinical documentation (treatment plans, progress notes).
Must complete a background check and LifeScan clearance. Arrestee and conviction records will be considered in the hiring process.
