Overview

Health Care Connectors is seeking a compassionate and skilled Home Health Nurse Practitioner to join our growing clinical team. The ideal candidate is clinically strong, organized, and passionate about delivering high-quality, patient-centered care in the home setting. Responsibilities

Perform comprehensive patient assessments in the home, including physical exams, medication reviews, and chronic disease management. Act as the primary care provider for patients who do not currently have a PCP, ensuring continuity of care and ongoing clinical oversight. Complete and maintain OASIS documentation accurately and on time (required). Develop, implement, and evaluate individualized patient treatment plans. Collaborate with RNs, therapists, physicians, and other interdisciplinary team members to coordinate care and improve patient outcomes. Provide education to patients and families to support disease management and promote independence. Participate in in-office case conferences, training, and team meetings (approx. 25% of workload). Utilize Homecare Homebase (HCHB) for documentation and workflow (preferred). Ensure compliance with all federal, state, and agency standards and regulations. Provide on-call support if needed (optional—customize as needed). Qualifications

Active, unrestricted Nurse Practitioner license in Connecticut (CT) . National certification (FNP, AGNP, or equivalent). Minimum 1–2 years of clinical experience ; home health experience strongly preferred. OASIS experience required. Homecare Homebase (HCHB) experience preferred. Excellent clinical judgment, communication, and organizational skills. Valid driver’s license, reliable vehicle, and ability to travel throughout the Connecticut service area. Ability to work independently in a field-based environment. Job Type: Full-time
