Physical Therapist – CareOne
Location: Paramus, NJ
Facility: CareOne at Ridgewood Avenue
Salary range: $40.00 - $63.00 per hour
Practice setting: Short Term Rehab, Long Term Care
Employment type: Full‑time
Responsibilities
Provide and direct resident rehabilitation care using accepted practice methods, ensuring compliance with all center, state, federal, and accrediting standards.
Evaluate resident needs and develop effective, efficient treatment plans for assigned residents.
Coordinate and supervise treatment plans with interdisciplinary staff and other disciplines as required.
Participate in resident screening according to directed schedules and practices.
Provide clinical and rehabilitation feedback to the designated supervisor.
Participate in all center‑directed processes and services.
Perform necessary clinical documentation to meet center, state, federal, and accrediting agency requirements.
Maintain complete, accurate, and timely billing records to meet center, state, federal, and accrediting agency requirements.
Participate in in‑service training programs as requested.
May travel to other facilities to provide services on a limited basis.
Collaborate daily with nursing and clinical staff to strengthen patient outcomes.

Qualifications
Master’s or Doctorate Degree in Physical Therapy.
Current and active Physical Therapist license in the state of operations.
Prefer experience or knowledge in older adult rehabilitation.
Prefer knowledge of governmental and regulatory guidelines in designated healthcare settings.
