Overview We are seeking a dedicated and compassionate Primary Care Nurse Practitioner/Physician Assistant to join our healthcare team. Responsibilities Conduct thorough patient assessments, including medical history and physical examinations. Develop and implement individualized care plans based on patient needs. Perform diagnostic tests and interpret results to guide treatment decisions. Administer treatments, including IV insertion and tube feeding, as necessary. Manage patient care in various settings, including memory care facilities and teaching hospitals. Participate in discharge planning to ensure smooth transitions of care for patients. Maintain accurate medical records and documentation for all patient interactions. Engage in utilization review processes to optimize patient outcomes and resource use. Provide education and support to patients and their families regarding health management. Skills Strong clinical skills in internal medicine and primary care practices. Proficient in venipuncture techniques and other essential nursing procedures. Excellent communication skills for effective patient interaction and education. Ability to work collaboratively within a multidisciplinary team environment. Knowledge of discharge planning processes and memory care considerations. Familiarity with medical record management systems. Strong organizational skills with attention to detail in patient documentation.
