We are seeking a friendly and dedicated Primary Care Physician to join a growing PACE organization in Springfield, MA.
Responsibilities

Under supervision of the Medical Director, provide primary care to frail older adults, including assessments, care planning, and medical procedures, collaborating with an interdisciplinary team.
