Primary Care Provider, Nurse Practitioner / Physician Assistant – Full‑Time

Overview WelbeHealth is a mission‑driven interdisciplinary team dedicated to providing life‑changing, community‑based care to our most vulnerable seniors. Our value‑based PACE program allows Primary Care Providers to deliver truly comprehensive, holistic care while building deeper, long‑term relationships with fewer patients than in a traditional healthcare model.
Responsibilities
Provide primary care for elderly Medicare/Medi‑Cal participants, including preventive, acute, and chronic disease management.
Prescribe medications, complete DEA registration, and maintain prescriptive authority.
Coordinate comprehensive, holistic care with interdisciplinary team members, including home visits, and participate in rotating on‑call schedule as needed.
Maintain accurate, compliant electronic health records and documentation per program and regulatory standards.
Qualifications
Licensed Nurse Practitioner or Physician Assistant with at least two years of experience working with the elderly.
Prescriptive authority, DEA registration, and eligibility to serve Medicare/Medi‑Cal participants.
Strong communication skills, ability to build trust with seniors and families, and commitment to a teambased model of care.
Valid California professional license and current registration with relevant certifying agencies.
Benefits
Compensation range: $145,399 – $174,478 per year + quarterly bonus + equity grants.
