Mileage Reimbursement $0.48 per mile
Position Summary The RN Case Manager is responsible for providing comprehensive care coordination for high-risk members. This role focuses on clinical assessment, care planning, care transitions, and patient advocacy while ensuring cost-effective, appropriate levels of care. The RN Case Manager collaborates closely with providers, interdisciplinary teams, and community resources to support optimal patient outcomes.
Primary Responsibilities
Perform comprehensive assessments of high-risk members
Collaborate with primary care providers to develop and implement individualized care plans
Continuously assess and adjust levels of care to ensure cost-effective and appropriate treatment
Identify and arrange non-medical supports (e.g., housing, transportation) to promote treatment compliance
Coordinate care transitions across settings
Collaborate with physicians, social workers, discharge planners, and claims professionals
Engage specialty resources as needed to achieve optimal outcomes
Maintain detailed documentation of clinical, functional, and financial outcomes
Identify opportunities for health promotion and illness prevention
Prevent adverse patient outcomes when possible and intervene promptly when prevention is not possible
Act as a patient advocate while maintaining privacy and confidentiality standards
