REGISTERED NURSE (RN) – CASE MANAGER
Location:
Brunswick, GA 31520 Pay
: $56.14 Schedule: Monday – Friday | 8:00 AM – 5:00 PM Full-Time On-call required one (1) week per month

Georgia RN license or compact license
Mileage Reimbursement:
$0.48 per mile
Counties Covered:
Charlton, Camden, Glynn, Brantley
JOB SUMMARY The RN Case Manager is responsible for performing comprehensive assessments and coordinating care for high-risk members to ensure cost-effective, appropriate, and patient-centered outcomes. This role partners closely with healthcare providers, community resources, and interdisciplinary teams to implement individualized care plans, manage transitions of care, and promote optimal health outcomes while advocating for patient privacy and confidentiality.
PRIMARY RESPONSIBILITIES Perform comprehensive assessments of high-risk members
Collaborate with primary care providers to develop and implement individualized, comprehensive care plans
Continuously assess care plans to ensure cost-efficient and appropriate levels of care
Coordinate care transitions and engage specialty resources as needed
Identify and arrange non-medical support services such as housing and transportation to support treatment compliance
Coordinate with physicians, social workers, discharge planners, and claims professionals
Maintain detailed documentation of clinical, functional, and fiscal outcomes
Identify opportunities for health promotion and illness prevention
Prevent adverse patient outcomes when possible and intervene promptly when prevention is not possible
Serve as patient advocate while maintaining privacy and confidentiality standards
PREFERRED SKILLS Wound care
Catheter care
IV antibiotics
Lab coordination
Drain management
Experience with HCHB (Homecare Homebase) software
QUALIFICATIONS Associate’s or Bachelor’s Degree in Nursing or related field
Current, unrestricted Registered Nurse license
Ability to analyze complex medical information and make sound clinical decisions
Case Management certification required
