Field RN Case Manager Compensation Pay rate:

$56.14 per hour

Mileage reimbursement: $0.49 per mile

Benefits available to full-time employees after 90 days

401(k) with company match available after 1 year of service

Field RN Case Manager Highlights Contract-to-hire opportunity

Full-time, Monday–Friday

Shift:

8:00 AM – 5:00 PM

Monthly on-call rotation

Onsite/field-based role

Field RN Case Manager Responsibilities Perform comprehensive assessments of high-risk members

Collaborate with primary care providers to implement individualized care plans

Identify and reassess cost-effective, appropriate levels of care

Coordinate care transitions with physicians, social workers, discharge planners, and other healthcare professionals

Arrange non-medical support services such as housing and transportation to support treatment compliance

Engage specialty resources to achieve optimal patient outcomes

Maintain detailed clinical, functional, and fiscal documentation

Identify opportunities for health promotion and illness prevention

Act as a patient advocate while protecting privacy and confidentiality

Field RN Case Manager Requirements Active, unrestricted

Illinois RN license

Associate’s or Bachelor’s Degree in Nursing or related field

Ability to analyze complex medical information and make sound clinical decisions

Attendance is mandatory for the first 90 days

Field RN Case Manager Preferred Qualifications Case Management certification

Home health experience with strong clinical competency

Experience with infusion, labs, OASIS, admissions, recertifications, resumptions of care

Wound care, wound vacs, ostomies, tube feeds

Familiarity with Homecare Homebase

Coverage area includes: St. Clair, Madison, Clinton, Randolph, Bond, Monroe, and part of Washington County
