Compensation Range:

$21/hr - $24/hr commensurate with experience

Benefits:

We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Overview:

This position is responsible for identifying, cultivating, and developing long‑term relationships with 25‑50 referral sources or community partners, with the goal of serving the needs of the local ENY community by connecting them to Housing Works services including primary care, OASAS, OMH, harm reduction, and HCV services. The Account Manager will be skilled at listening and understanding their community partner’s priorities and creating value by responding with solutions and services. The role acts as a brand ambassador to uphold and advance the mission and services of Housing Works with integrity. As an outreach role, the Account Manager is accountable for meeting monthly targets and increasing Housing Works’ client base to deliver quality care to those in need. The right candidate will be sensitive, flexible and responsive to gender, race, ethnicity, socioeconomic status, religion, age, sexual orientation or any special needs in interactions with staff, clients and referral sources.

Responsibilities:

Represent Housing Works in a professional manner in the community and at networking events.

Master product knowledge: understand Programs and Services, especially the Hepatitis C Care and Treatment initiative, and educate partners about eligibility criteria, service offerings and referral process.

Map assigned territory and identify stakeholders in Brooklyn who serve clients with elevated risk for HCV (methadone clinics, substance‑use inpatient/outpatient facilities, recovery centers) and develop a territory plan that optimizes time management.

Profile accounts, understand customer needs, set SMART goals and create solutions to differentiate Housing Works and advance relationships.

Prospect community‑based organizations and partners that serve individuals in need of HCV, primary care and behavioral health services.

Segment the market, tailor strategy by referral channel and build an action plan to achieve targets.

Apply prioritization criteria to focus on high‑value accounts.

Meet documentation requirements and use the CRM to manage the account portfolio.

Understand referrer or client value, handle objections, communicate with key influencers and negotiate strategically.

Deliver strategic solutions, achieve Triple WIN, and provide follow‑up to advance relationships.

Participate in Community Partnerships and Prevention Services team meetings, collaborating with both departments.

Provide excellent internal and external customer service to create a superior experience for referrers and clients.

Obtain agreement to connect clients to Hepatitis C treatment and other Housing Works programs.

Track daily and monthly activity, record metrics, understand account trends and provide weekly reports.

Present educational workshops and presentations, engaging participants in valuable dialogue.

Conduct HCV/HIV rapid testing for clients at partner sites that express a desire to know their status.

Meet performance standards related to call reach, visit frequency and required documentation.

Requirements:

High School Diploma or equivalent with at least 3 years of related work experience, OR

Bachelor’s Degree with at least 1 year of related work experience.

Developed knowledge of social services organizations and community outreach efforts; experience in outreach, sales, or business development is a plus.

Personal qualities of empathy, integrity, and a sense of responsibility and urgency.

Valid driver’s license preferred.
