Job Description Job Description We are seeking a compassionate and experienced Certified Nursing Assistant (CNA) to provide high-quality, patient-centered care to individuals in their homes or a residential setting. The ideal candidate will have a minimum of two (2) years of experience in either home health or hospice care, demonstrating a strong commitment to patient comfort, dignity, and support during critical times. This role involves working closely with an interdisciplinary team to ensure a comprehensive and supportive care plan for each patient and their family. Key Responsibilities: Personal Care Assistance: Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, oral hygiene, and toileting, while maintaining their dignity and ensuring their comfort. Monitoring and Reporting: Measure and record vital signs (blood pressure, pulse, temperature, respiration) and track food/fluid intake and output. Promptly report any changes in the patient's condition or behavior to the supervising Registered Nurse (RN) or the interdisciplinary team. Mobility Support: Safely assist patients with ambulation, transfers (e.g., bed to wheelchair), and repositioning to prevent bed sores and promote comfort. Emotional and Social Support: Provide empathetic companionship and emotional support to patients and their families, addressing their concerns and fostering a sense of peace and well-being during end-of-life care. Environment Management: Perform light housekeeping tasks related to patient care, such as changing bed linens, tidying the patient's immediate area, and ensuring a safe and clean environment. Documentation and Communication: Maintain accurate patient records and documentation of all care provided in accordance with federal and state regulations and company policy. Care Plan Collaboration: Follow the established care plan developed by the RN case manager and actively participate in patient care conferences and team meetings. Logistics: May assist with meal preparation (according to dietary needs), grocery shopping, or arranging transportation to appointments in a home health setting. Qualifications and Skills: Experience: Minimum of two (2) years of documented experience as a CNA in a home health or hospice setting is required. Certification/Licensure: Current, active Certified Nursing Assistant (CNA) certification in the state of Texas Current CPR certification is required. Valid driver's license, reliable transportation, and current auto liability insurance are essential for home visits. Education: High school diploma or equivalent. Skills: Demonstrated knowledge of end-of-life care principles and patient safety protocols. Exceptional communication and interpersonal skills, with a patient and empathetic approach. Strong observational skills and attention to detail. Physical stamina to perform duties, including lifting and transferring patients (ability to lift greater than 50 lbs is often required). Ability to work independently in various home environments and adapt to changing patient needs. Integrity and a commitment to maintaining confidentiality (HIPAA). Work Environment: Work is performed in patients' private residences, which may have varying conditions. The role requires local travel and the ability to work a flexible schedule, which may include evenings, weekends, or on-call shifts.
