Social Worker - Care Manager Overview:

The Social Worker - Care Manager delivers clinical social work services, which encompass psychosocial evaluations, treatment strategies, crisis management, and discharge planning for individuals in acute medical or mental health care environments. This role involves collaboration with patients, their families, and healthcare teams to meet social and emotional needs while optimizing healthcare resource utilization. Candidates must possess a Master's degree in Social Work, relevant licensure, and experience in either inpatient or outpatient healthcare settings.

Position Details

This role operates during Night Shifts.

Eligible external hires may receive a $5,000 Hiring Bonus, contingent upon meeting specific qualifications and payment conditions.

Care Manager MSW Responsibilities

The Clinical Social Worker collaborates with patients, families, and the healthcare team to advocate for and address patients' social and emotional requirements. Responsibilities include a comprehensive range of social work services such as psychosocial assessments, treatment planning, therapeutic interventions, discharge planning, crisis management, and resource referrals. The Clinical Social Worker works closely with patients and their support systems, as well as interdisciplinary teams, to enhance patient care through effective resource utilization and monitoring. This role also involves leadership in achieving patient goals and desired clinical, financial, and resource outcomes, guided by the NASW Code of Ethics and the organization's Mission and Vision.

Essential Qualifications:

  • Master's Degree in Social Work from an accredited institution.
  • Upon hire: Washington Clinical Independent Social Worker Associate License.
  • Minimum of 1 year of experience in acute medical or mental healthcare settings, either inpatient or outpatient.

Salary Range: Minimum: $45.66, Maximum: $69.47

Preferred Qualifications:

  • ACM/CCM certification.

Care Manager LCSW Responsibilities

The Clinical Social Worker engages with patients, families, and the healthcare team to advocate for and address patients' social and emotional needs. Responsibilities mirror those of the MSW role, including psychosocial assessments, treatment planning, therapeutic interventions, discharge planning, crisis management, and resource referrals. The Clinical Social Worker collaborates with patients and their support systems, as well as interdisciplinary teams, to enhance patient care through effective resource utilization and monitoring. Leadership in achieving patient goals and desired clinical, financial, and resource outcomes is also a key aspect of this role, adhering to the NASW Code of Ethics and the organization's Mission and Vision.

The salary range provided reflects the base pay for this position. The successful candidate's placement within this range will be influenced by relevant experience and other applicable factors. Additional compensation may be available for this role, including shift differentials, standby/on-call pay, overtime, premiums, extra shift incentives, or bonuses.
