Care Manager - Social Worker Overview:

Care Manager MSW

The Clinical Social Worker collaborates with patients, families, and the healthcare team to advocate for and address patients' social and emotional requirements. Responsibilities include providing a comprehensive range of social work services such as psychosocial assessments, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referrals. The Clinical Social Worker works closely with patients and their support systems, as well as interdisciplinary teams, both within and outside the organization, to enhance patient care through effective resource utilization and monitoring. This role also involves leadership in achieving patient goals and desired clinical, financial, and resource outcomes, guided by the NASW Code of Ethics and the organization's mission and vision.

Required Qualifications:

  • Master's Degree in Social Work from an accredited institution.
  • Upon hire: Washington Clinical Independent Social Worker Associate License.
  • Minimum of 1 year of experience in acute medical or mental healthcare settings, either inpatient or outpatient.

Salary Range: $45.66 - $69.47

Preferred Qualifications:

  • ACM/CCM certification.

Care Manager LCSW

The Clinical Social Worker in this role also partners with patients, families, and the healthcare team to advocate for and address patients' social and emotional needs. Responsibilities mirror those of the MSW role, including psychosocial assessments, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referrals. The Clinical Social Worker collaborates with patients and their support systems, as well as interdisciplinary teams, to enhance patient care through effective resource management. Leadership in achieving patient goals and desired clinical, financial, and resource outcomes is also a key aspect of this role, guided by the NASW Code of Ethics and the organization's mission and vision.

Required Qualifications:

  • Master's Degree in Social Work from an accredited institution.

  • Minimum of 1 year of experience in acute medical or mental healthcare settings, either inpatient or outpatient.

Salary Range: $48.39 - $73.64
