Salary and Benefits
- Salary range: $140,000+ (based on experience)
- 10% annual bonus (based on metrics such as hospital sick days, HEDIS scores, patient satisfaction, and alignment with company values)

Responsibilities
- Outpatient primary care practice (Internal Medicine, Family Medicine, Geriatrics, Preventive Medicine)
- No inpatient or on-call duties
- Patient panel capped at 650 for optimal quality and continuity of care
- Average of only patients per day, allowing for comprehensive, unrushed visits
- Perform in-office medical procedures, including:
- Suturing for minor wound closures
- Joint injections for pain management
- Skin biopsies for diagnostic purposes
- Comprehensive wound care
- Preventive screenings relevant to geriatric patients
- Collaborate within a multidisciplinary team
- Supported by a dedicated care team to enhance provider-patient interaction
- Practice in newly built, community-based centers using cloud-based EHR technology
Requirements
- Nurse Practitioner degree from an accredited program
- Active, non-probationary Arizona Nurse Practitioner license
- Minimum 2 years of primary care outpatient experience required
- Proficient in minor in-office procedures (suturing, joint injections, skin biopsies, wound care, preventive screenings)
- Training in geriatrics welcome, but not required
- Highly collaborative and committed to a patient-centered approach
- Strong passion for serving elderly, medically complex, and underserved populations
- Confident leader and team player, dedicated to providing exceptional primary care
