Job Description Overall Responsibility: Administers therapy services to patients requiring intermittent physical therapy service and teaches and/or supervises the family and other members of the home health team. These services are performed in accordance with the physician’s orders.

Duties Include:

Responsible for supervising/training personnel such as PTAs or HHAs.

Following receipt of doctor’s referral, the therapist is responsible for evaluating the patient within 72 hours of written referral.

Will provide therapy services as outlined in the Plan of Care.

After evaluating the patient, the therapist is responsible for planning, administering and supervising the PTA and HHA in accordance with the treatment plan.

Will communicate with the referring physician and members of the rehabilitation team regarding the patient’s total treatment program.

Will maintain accurate daily records of treatments given to patients.

Will provide in-service education to facilities, support staff, students and community.

Will follow established policies and procedures of Nightingale Home Healthcare.

Charts those services the Therapist has rendered to the patient, notes changes in the patient’s condition and/or family and home situation, makes revisions in the care plan as needed, records supervisory visits, evaluates patient care and progress and closes charts of discharged patients by preparing Discharge Summaries and obtaining Discharge orders.

Maintains confidentiality of client’s situation, records and services provided according to HIPAA regulations.

Evaluates the effectiveness of his/her therapy service to the individual and family.

Consults with the attending physician concerning alterations of the plan of treatment.

Submits all patient notes and/or visit records on a timely basis.

Discusses with the Director of Clinical Services problems concerning the patients and how these may best be resolved.

Discusses with the Director of Clinical Services changes in patient needs and how these can be best met.

Discusses with the Director of Clinical Services the need for involvement of other members of health team.

Obtains orders for paramedical services and submits referral to the Director of Clinical Services (i.e., nursing services, medical social services, etc.).

Coordinates total patient care.

Cooperates with other agencies providing nursing or related services to provide continuation of care and to implement a comprehensive care plan.

Continually strives to improve therapy care by broadening and deepening knowledge through formal education, attendance at workshops, conferences and active participation in professional and related organizations and reading.

Participates in interdisciplinary team conferences.

Attends on-the-job and in-service training.

Qualifications

Education: Licensed Physical Therapist in the state of California.

Experience: Physical therapy experience preferred.

Additional Requirements:

Physical exam within 6 months prior to first patient contact.

Current TB results (within last 12 months).

Current CPR Certification.

Must have a valid driver’s license.
