MDS Coordinator

Job Summary We are seeking a detail-oriented and experienced MDS Coordinator to join our team at Lakeland located in Southfield, MI. The ideal candidate will be responsible for coordinating and managing the Minimum Data Set (MDS) assessments to ensure compliance with federal and state regulations, while supporting quality resident care.
Key Responsibilities
Coordinate and complete MDS assessments accurately and timely in accordance with CMS guidelines.
Collaborate with interdisciplinary team members including nursing, therapy, and medical staff to gather necessary information for assessments.
Review resident records and clinical data to ensure accuracy and completeness of MDS documentation.
Monitor and maintain compliance with federal, state, and facility regulations related to MDS and reimbursement.
Assist in the development and implementation of care plans based on MDS data.
Provide education and training to staff regarding MDS processes and documentation requirements.
Communicate effectively with residents, families, and healthcare team members regarding care planning and assessment outcomes.
Utilize electronic health record (EHR) systems to document and track MDS data.
Qualifications
Proven experience as an MDS Coordinator, preferably in a skilled nursing or long-term care setting.
Strong knowledge of CMS MDS guidelines, regulations, and reimbursement processes.
Excellent organizational and communication skills.
Ability to work collaboratively within an interdisciplinary team.
Proficiency with electronic health records and MDS software systems.
Attention to detail and strong analytical skills.
Current nursing license RN
