Job Overview As a full‑time Housing Stabilization Specialist, you will play a vital role in transforming the lives of individuals with intellectual and developmental disabilities, substance abuse disorders, serious mental illnesses, and co‑occurring disorders. You will collaborate with clients to identify their unique housing needs, create comprehensive housing plans, and provide supportive counseling and in‑home skill support to facilitate secure, stable housing and greater independence.
Responsibilities
Identify clients’ housing needs and coordinate housing solutions.
Develop and implement comprehensive housing plans.
Provide in‑home, housing‑focused support and counseling.
Maintain accurate records in the Electronic Health Record (EHR) system.
Build rapport with clients, landlords, and community partners.
Transport clients as needed (valid driver’s license required).
Collaborate with multidisciplinary teams to address client needs.
Qualifications
Bachelor’s Degree in Human Services (Social Work, Psychology, Sociology, Counseling, or related field).
One (1) year of case‑management experience, including housing resources for adults with mental health, developmental disabilities, or substance‑use disorders.
Valid driver’s license and ability to transport clients.
Proficiency with case‑management software and strong documentation skills.
Exceptional communication, interpersonal, and relationship‑building skills.
Background checks: criminal history fingerprint, DMV record, Child Registry, drug screening, and reference checks.
